Synopsis:

Requiring uniform claims forms to include presenting symptoms under specified circumstances; altering the circumstances under which additional medical information may be requested by health maintenance organizations (HMOs), insurers, and nonprofit health service plans; requiring HMOs, insurers, and nonprofit health service plans to reimburse a hospital emergency facility or health care practitioner under specified circumstances within 30 days after receipt of a claim for reimbursement for emergency services; etc.
