Requiring a health insurance carrier or its credentialing intermediary to provide specified notifications regarding a credentialing form to a health care provider; requiring a carrier or its credentialing intermediary to complete the credentialing or recredentialing process within a specified period of time; requiring a carrier to continue to credential a health care provider under specified circumstances; requiring a written notice to include specified information; etc.
