Patient Centered Medical Home Program

Synopsis:

Requiring the Maryland Health Care Commission to establish a Maryland Patient Centered Medical Home Program under specified circumstances; requiring specified health insurance carriers to participate in the Program; requiring the Department to ensure that participation in the Program of managed care organizations and specified enrollees will support specified standards; authorizing the Commission to authorize a health insurance carrier to implement a specified single carrier patient centered medical home program; etc.
