Requiring a carrier or its credentialing intermediary to accept the uniform credentialing form established by regulation as the sole application of a health care provider to be credentialed or recredentialed by the carrier or its credentialing intermediary for participation on a provider panel of the carrier; requiring a carrier or its credentialing intermediary to make available the uniform credentialing form to a health care provider that is to be credentialed or recredentialed; etc.
