Synopsis:

Requiring an insurer or a person who has a self-insurance plan to provide information on the last known home address, if known, of an insured defendant under specified circumstances; providing civil and criminal immunity to a person who discloses information required under the Act; and providing that service may be effected by delivering a copy of a summons and complaint at a defendant's dwelling house or usual place of abode under specified circumstances.
