Medical Records - Disclosure to Health Care Provider's Insurer or Legal Counsel

Synopsis:

Amending certain provisions that authorize disclosure of a medical record to a health care provider's insurer or legal counsel for the purpose of handling certain types of claims against the provider to require that a request for disclosure of a medical record be accompanied by a copy of a certificate that notifies the patient, recipient, claimant, or person in interest or counsel for the patient, recipient, claimant, or person in interest that disclosure of the information is being sought; etc.
