Prohibiting insurers, nonprofit health service plans, and health maintenance organizations from limiting or excluding coverage for prescription drugs if an authorized prescriber prescribes a prescription drug not on a drug formulary after the authorized prescriber makes a determination that the drugs included in the drug formulary have been ineffective or continues to prescribe a prescription drug that was previously approved for coverage.
