Health Insurance - Coverage for In Vitro Fertilization
Prohibiting specified insurers, nonprofit health service plans, and
health maintenance organizations from excluding specified benefits for
in vitro fertilization under specified circumstances; providing that,
to be eligible for in vitro fertilization health insurance benefits,
the patient and the patient's spouse have a history of infertility of
at least 2 years' duration or the infertility is associated with
abnormal male factors contributing to the infertility; requiring that
the benefits be provided to specified extents; etc.
