Health Insurance - Coverage for Hair Prosthesis for Hair Loss Resulting from Alopecia

Synopsis:

Requiring insurers, nonprofit health service plans, and health maintenance organizations to provide coverage for a hair prothesis for hair loss resulting from alopecia if the cost does not exceed $350 and the hair prosthesis is prescribed by the attending physician of the insured or enrollee being treated for alopecia.
