Synopsis:

Requiring each child in the aftercare program in the Department of Juvenile Justice to be assigned to an aftercare team; requiring each aftercare team to prepare an aftercare plan for each child in aftercare assigned to the team and file the plan with the juvenile court; requiring each aftercare team to provide specified reports to the Secretary of Juvenile Justice and the juvenile court; and requiring a specified report to include specified information.
