Workers' Compensation Commission - Authorization for Release of Medical Information - Work-Related Injury or Occupational Disease

Synopsis:

Requiring specified claim application forms that are filed with the Workers' Compensation Commission for an alleged work-related injury or occupational disease to include an authorization for the release to specified persons of specified medical information; providing for the contents, duration, and use of the authorization; requiring a health care provider to disclose specified medical information on receipt of a specified authorization filed with the Commission; etc.
