Synopsis:

Requiring the State Department of Education to require local school systems to utilize passive parental consent before administering the Centers for Disease Control and Prevention Youth Risk Behavior Surveillance System survey; requiring local school systems to provide parents with specified statements and forms; requiring that for the academic year during which the survey will be conducted, denial of permission forms be part of a request for emergency contact information; etc.
