Living at Home Waiver Program - Case Management - Eligibility

Synopsis:

Requiring nursing facilities to notify a case management entity when individuals are admitted under specific circumstances; requiring nursing facilities to allow representatives of a case management entity access to nursing facilities; providing for the development of a transition plan if individuals are eligible for a specified program; and requiring a case management entity to provide outreach and training to nursing facility operators and staff.
