Department of Health and Mental Hygiene - Forms - Social Security Number

Synopsis:

Prohibiting a specified form provided by the Secretary of Health and Mental Hygiene from requiring or requesting an individual's complete Social Security number; and authorizing a specified form provided by the Secretary to request the last four digits of an individual's Social Security number for a specified purpose.
