Requiring specified health care facilities to provide specified
information concerning the right to make decisions about health care,
request a copy of existing advance directives under specified
circumstances, and inform specified individuals or proxy decision
makers about facility procedures; requiring a health care facility to
disclose the facility's policy on honoring advance directive
provisions in a specified manner; etc.
