Requiring an adult individual to sign a written statement acknowledging specified facts and warnings about the use of tanning devices before using a tanning facility; requiring a parent or legal guardian of a minor individual to sign a written statement acknowledging specified certain facts and warnings about the use of tanning devices before the minor may use a tanning facility; requiring specific language to be included in any written statement that is required to be signed under the Act; defining certain terms; etc.
