Synopsis:

Prohibiting specified carriers from requiring health care providers that provide health care services through a specified group practice or health care facility to be considered participating providers or to accept a specified reimbursement fee schedule under specified circumstances; and requiring a specified provider to give specified notice to an enrollee.
