Requiring insurers, nonprofit health service plans, and health maintenance organizations to provide insureds with insurance identification cards; requiring insurers, nonprofit health service plans, and health maintenance organizations to ask insureds if they have living wills; and requiring specified information to be on the insurance identification card; etc.
