Synopsis:

Increasing the number of fees on a schedule of specified services billed by specified health care practitioners that health insurance carriers must provide health care practitioners in a specified manner; and requiring health insurance carriers to provide health care practitioners with a pharmaceutical formulary in a specified manner.
