Workers' Compensation - Claim Application Form

Synopsis:

Requiring an employer or its insurer to inform employees or their personal representative of their right to file a workers' compensation claim application form under specified circumstances; establishing that a specified notice and application form be provided; establishing specified sanctions for failure to comply with the Act; and requiring the Workers' Compensation Commission to alter or develop a specified form.
