Requiring health care providers and health care facilities to require
a written advance directive for the disposition of cryopreserved eggs,
sperm, or embryos following a specified format and including specified
options; authorizing the inclusion in an advance directive of options
under specified circumstances; and providing a limited immunity from
civil, criminal, and disciplinary actions when an advance directive is
followed.
