Authorizing a nonhospital-based health care provider that is not under written contract with a health maintenance organization (HMO) to collect or attempt to collect from a subscriber or enrollee of the HMO payment for nonemergency health care services under certain circumstances; requiring the Insurance Administration to develop the format and content of a certain waiver form; etc.
