Health Maintenance Organizations - Hold Harmless Clause - Balance Billing - Authorization

Synopsis:

Authorizing a health services provider to bill, charge, collect a deposit from, seek compensation, remuneration, or reimbursement from, or have any recourse against specified individuals for covered services if the provider is not under contract with the health maintenance organization and the provider notifies the patient of the lack of a contractual relationship between the provider and the health maintenance organization.
