Requiring insurers, nonprofit health service plans, and health
maintenance organizations to provide coverage for prosthetic devices,

copayment or coinsurance requirement; prohibiting insurers, nonprofit
health service plans, and health maintenance organizations from
imposing a specified dollar maximum on specified coverage; etc.
