Health Care Decisions Act - "Medical Orders for Life-Sustaining Treatment" Form
Requiring the Department of Health and Mental Hygiene, in conjunction
with the Maryland Institute for Emergency Medical Services Systems and
the State Board of Physicians, to develop and periodically revise a
"Medical Orders for Life-Sustaining Treatment" form; requiring
specified health care facilities and authorizing other health care
providers to use a "Medical Orders for Life-Sustaining Treatment"
form; repealing provisions of law relating to the "Instructions on
Current Life-Sustaining Treatment Options" form; etc.
